Eligibility:

 

 

 

·         travel for religious or missionary purposes;

 

 

 

Required documents (the Consular Officer processing the visa can ask for additional documents if deemed necessary):

1)   Two (2) receipt pages from visa application form per applicant, properly and completely filled in and signed by the applicant;

2)   A passport valid for at least 6 months prior to its expiration date, and with at least one blank page available for the visa stamp;

3)   two recent passport-size photo, front view, full face, either in color or black and white, with a light background (snapshots are not accepted);

4)   Visa fee, payable in US Postal Money Order only, as applicable (see visa fees table); A non-refundable handling fee of US$20.00 per visa applies to visa applications submitted by mail or by any individual other than the applicant or an immediate member of his or her family.

5)   ordination certificate and/or diploma and school transcript for complete undergraduate theological studies;

6)   curriculum vitae;

7)   recent (i.e., less than 90 days old) non-criminal record issued by the (i.e., applicants place of residence) Police Department;

8)   marriage certificate, birth certificate or official dependency document for spouse/children/dependents (if applicable);

9)        proof of residence for at least one year in the consular jurisdiction;

10)        signed statement of commitment that the applicant will not engage in any activity in indigenous areas without the proper authorization by FUNAI - "National Indian Foundation";

11)        Brazilian notarized documents (send the documents with the original stamp by the "Cartório", plus one copy each):

12)        constitutive minutes/statutes of the Brazilian religious or missionary organization;

13)        empowerment certificate issued to the Brazilian religious or missionary organizations head officer;

14)        financial affidavit drawn at a "Cartório" by the Brazilian religious or missionary organization on behalf of the visitor and on his exit from the country;

15)   For non-Americans: copy of green card or valid re-entry visa into the USA or valid visa to country of final destination;

International Certificate of Yellow Fever Immunization is RECOMMENDED, BUT NOT REQUIRED, if the applicant has traveled, within the last 90 days, to any of the following countries: Angola, Benin, Bissau Guinea, Bolivia, Burkina Faso, Burundi, Cabo Verde, Camerun, Central African Republic, Colombia, Congo, Democratic Republic of Congo, Ecuador, Equatorial Guinea, Ethiopia, French Guiana, Gabon, Gambia, Ghana, Guiana, Guinea, Ivory Coast, Kenia, Liberia, Mali, Mauritania, Niger, Nigeria, Panama, Peru, Rwanda, Sao Tome & Principe, Senegal, Sierra Leone, Somalia, Sudan, Surinam, Tanzania, Togo, Trinidad and Tobago, Uganda or Venezuela.

International Certificate of Yellow Fever Immunization is advisable if applicants destination in Brazil includes any of the following States: Acre, Amapá, Amazonas, Federal District, Goiás, Maranhão, Mato Grosso, Mato Grosso do Sul, Pará, Rondônia, Roraima and/or Tocantins.
